PARENTAL ACKNOWLEDGEMENTS FOR SCHOOL-AGE
CHILD CARE PROGRAM, BEFORE & AFTER SCHOOL PROGRAMS

Each participating parent must acknowledge in writing having received, read, and acknowledged the Parent Handbook and Payment Schedule. Each parent and child agrees to abide by the policies set forth for the school-age child care program.

  • Fees must be paid on time and in full based on the payment schedules. Failure to make payments may result in the child being withdrawn from the program.
  • Late payment fee $10 for payments not received prior to the first day ofthe service period.
  • Late pick up fee $10 per 15 minutes per family
  • Any returned checks and bank service charges must be paid in cash within 24 hours ofnotification or the child may be withdrawn from the program.

Program
A variety of activities, including homework assistance, indoor. and outdoor games, music, and arts and crafts have been planned for your child's time in our program.

Snacks
Each day a snack will be provided for your child after the end of the regular school day. Please notify staff of any food allergies upon registration. lf your child is unable to eat the snacks provided by the school-age child care program, please contact our office. You will be responsible for providing a nutritious snack

Alternate Nutrition Plan
]f your child is unable to eat the snack provided by the after-school care program, please contact our office. There is a form to complete that is kept in your child's file stating that the parents will provide a nutritious snack.

Registration
Registration cards and emergency contact cards must be completed in full. No registration card is considered complete without EMERGENCY CONTACT INFORMATION. All infonnation on the card must be filled in. It is EXTREMELY IMPORTANT that you notify the program manager or the community school specialist if there are any changes in the parent contact and/or emergency contact.

School Insurance
Upon registering your child, it is mandatory to obtain the student accident insurance. NO CHILD MAY ATTEND THE BEFORE & AFTER-SCHOOL CARE PROGRAM WITHOUT THE SCHOOL INSURANCE. This is supplemental insurance and DOES NOT take the place of family or individual medical insurance coverage. It is the responsibility of each parent to become familiar with any insurance limitations and other infonnation provided in the brochure.

Fees
Fees must be paid in advance, on a monthly basis, based on the payment schedule specified in the Parent Acknowledgment Form. Fees may be paid by cash, check or money order. There will be no partial payments for partial use of services. A fee schedule, showing the payment amounts and due dates will be provided for you at registration.

Before-School Care Arrival Procedures
Each child must be signed in daily within the school building by an authorized persons listed on the child's registration card.

Release of Children
Children may be picked up any time prior to 6:00 p.m., but they must be picked up NO LATER THAN 6:00 p.m. If an emergency arises. you are expected to make arrangements so that your child will be picked up before 6:00 p.m. and to notify the program manager accordingly.

Due to the excessive number of late pick-ups. we have to enforce a late pick-up charge. After 3 late pick-ups (after 6:00 p.m.), a child may be removed from our program.

Please be advised that our program is only designed to meet the needs of parents in our community that need their child(ren) to remain in school until 6:00 p.m. Therefore, if you anticipate any conflict with our schedule, you need to consider other means of after-school care.

Late Pick-Up Fee
A late pick-up fee of $10.00 per 15 minutes per family, will be charged-for children not picked up on time. Consistent late pick tip may result in the child being withdrawn from the program.

Dismissal Procedures

Pick-up - Each child must be signed-out from the after-school care office by an AUTHORIZED PERSON listed on the child's registration card or emergency contact card. Photo identification will be required until the staff is familiar with that person or persons. A pass will be issued to pick up the child from the after-school care classroom or outdoor play area. Any other person not listed must have permission in writing from the parents or legal guardian and present photo identification before the child will be released.

Authorized Persons - ONLY those persons listed on your child's registration card or emergency contact cards are considered authorized. Even family members, if not listed, are not considered authorized to pick up your child. Verification by the program manager or community school specialist will be made with you before any child is released to a person not listed on the registration card.

Walk Home - A written notification must be kept on file or sent the same day that your child is to walk home. The time of departure must be specified in the written notification. The program manager must approve the child's departure.

Changes in Procedures
If there are any changes in the dismissal procedures for your child, daily or continuous, please notify the office in person or in writing immediately. This will help to ensure the safety of your child. If you pick up your child early from the regular day school program, please notify the school-age child care office.
